hard copies of forms linked below. Final bill for Close Up needs to be paid on Wednesday at the Parent Meeting. In addition…all forms must be signed and turned in to Mr. Wood by Thursday morning. Finally, Mr. Wood will need a copy of medical insurance cards by Thursday morning….
